THE EXPANDING ROLE OF MRI IN PROSTATE CANCER: CURRENT STATUS AND FUTURE TRENDS

  Prostate cancer (PCa) is the second leading cause of death by cancer in men worldwide, and its incidence is expected to double by 2030 due to aging global population. Unlike other cancers types, there was no reliable  imaging modality  to detect PCa until a decade ago.  Therefore, when there was suspicion of PCa, the only definitive way to confirm it was to biopsy the gland randomly under the guidance of transrectal ultrasound [TRUS]. Since tissue samples are taken at random, this method is associated with overdiagnosis of low-malignant potential cancers, and underdiagnosis of potentially lethal cancers. Furthermore, like all invasive procedures TRUS guided biopsy carries a risk of infection.

  By the means of the recent developments in MRI technology along with the release of PIRADS (Prostate Imaging - Reporting and Data System) which outlines how to perform and interpret MRI studies,  multiparametric prostate MRI (mp-MRI) has been started to be used widely for the detection of PCa. This led to reduction of unnecessary biopsies and overdiagnosis. Also, since cancer became detectable by MRI, random TRUS guided biopsy is being replaced by ultrasound and MRI guided targeted biopsies which will also be discussed in this presentation.
